If your student is entering the 7th or 8th grade the following requirements apply:

VARICELLA VACCINE
2 doses of varicella vaccine. The first dose must have been given on or after the first birthday,
2 doses are required for any student who receives the first dose on or after 13 years of age
OR
a written statement from the parent or health care provider stating the student's history of the illness and the approximate date of the illness

MENINGOCOCCAL VACCINE
One dose of the meningococcal vaccine 


T-DAP VACCINE
One booster if at least 5 years have passed since the last tetanus-containing vaccine was received

will not be permitted to enroll or attend school
